Initial application fee to take the IV Insertion Exam as part of our IV Insertion Certification Validation Program.   Choose this option to apply to take the IV Insertion Exam.  Application fees are non-refundable.  Before submitting an application and fee; review the program requirements for successful completion that are found within the application package to ensure that you meet the program guidelines.  If you have not already received the application package; prior to submitting an order, contact us to request the application forms package by sending an email to customer_support@piccresource.com ; or call our office at (203) 377-5252 to have the forms emailed or faxed.  The forms are in pdf format.  To view the file you will need Adobe's pdf Reader program.  If you do not have Adobe reader, you can download a free program at http://www.adobe.com .  Upon completion of this order, a link to download the exam study guide will be automatically sent by our automated system to the email address provided within your oder.  The email is titled "Downloads Access", and it is sent immediately after processing of your order is completed.
